Please keep your first message and attachments general.Do not submit a diagnosis, date of birth, insurance member ID, medical records, detailed health information, government identification, or other sensitive records through the public website form.

When you use our inquiry form, we may collect contact information, a child or client’s first name, city, insurance provider name, contact and service preferences, a short message, and an optional PDF, JPG, JPEG, or PNG file. Career inquiries may include an applicant’s name, contact information, interests, resume, or general photo.

How we use information

We use the information to respond to service, referral, parent, career, and general inquiries; follow the contact method you select; and explain appropriate next steps.

Public form limits

This form is for initial contact and is not intended for urgent needs or detailed medical information. If additional records are needed, our team will explain an appropriate way to provide them.

Sharing and retention

Form submissions are emailed to authorized Badoni Behavioral Services LLC team members. The website form is not designed to create a public WordPress submission record. Email copies may be retained as needed to respond to and manage the inquiry. We may disclose information when required by law or to protect rights and safety.

Website services and translation

Website hosting, security, analytics, and similar service providers may process technical information such as browser type, device information, IP address, and pages visited. If you choose automated translation, the page opens through Google Translate and Google’s privacy terms apply. Automated translations may contain errors.

We use reasonable administrative and technical measures to protect information. No internet transmission or email system can be guaranteed completely secure, so please use the public form only for general first-contact information.

Children’s information

The service inquiry form is intended to be completed by a parent, guardian, adult client, or authorized referral source. Children should not submit information independently.
